Cost of Services

At Foundations Medical Center your health and well-being are our priority. Our financial policy allows us to provide you with access to the best treatment available without limiting your healthcare to what is covered by your health insurance policy.

We operate as a self-pay clinic and payment is expected at the time of your visit.  We accept cash, check, or credit card. 

Our office is not not a participating provider with commercial insurance companies, Medicaid or Medicare. We will  gladly provide an invoice at the conclusion of your visit so that you may send it to your insurance company for reimbursement of billable services for out of network services. IV infusions, injections, testosterone pellet therapy or the purchase of supplements are not billable services.

Fee schedule (Effective august 15, 2017):

For your initial consultation expect to spend at least 90 minutes at the office. You will spend 50-60 minutes of face-to face time with our providers. The consult fee includes time spent before and after your visit for our providers to review any past lab work and all paperwork and intake forms. Follow-up visits may last 30-45 min depending on complexity and time to review lab work.

Fee with Dr. Chavers:

  • Initial Consultation with Dr. Kyle Chavers: $325
  • Initial Consultation with Dr. Amanda Chavers $325
  • Follow-up Visit: $215
  • Phone Call Follow-up: $160
  • Acute Visit: $215

Other Fees : 

  • Injection Fee: $50
  • Vitamin B12 ( Methylated) Injection Fee: $50
  • MIC ( Weight loss) Injection Fee: $50
  • Testosterone Pellet Insertion Fee: $250-$500
  • CIMT testing: $250 (includes follow-up phone consult to discuss results)

  • IV Vitamin Drips and Hydration: starting at $100

  • Infrared Sauna sessions: $ 40 per session/ $120 for 1 month unlimited or $100 for 3 month membership

  • Initial Consultation with Elizabeth Trapp | Dietitian: $120

  • Follow-up Visit with Elizabeth Trapp | Dietitian: $ 60

 

Cancellation Policy
(EFFECTIVE JANUARY 1, 2017)

Due to the amount of time set aside in our schedule for each patient, cancellations must be made 72 hours prior to your appointment time. This helps ensure we are able to meet the needs of all our patients who are waiting for an appointment time.  A credit card will be required for your appointment to be reserved and you will be charged a $75 fee for cancellations made less than 72 hours in advance or in the case of a no-show. This applies to patients who have a reserved appointment with Dr. Kyle Chavers, Dr. Amanda Chavers, Elizabeth Trapp- Dietitian and in our IV Nutritional Therapy Clinic.
